I lost my vaccine record card. Can Curative give me a replacement?

Our Customer Care Team would be happy to provide you with a printable version of your vaccination receipt or a wallet-sized record card upon request. Check with your primary care physician to see if a replacement vaccine record card can be obtained through them. 

Unfortunately, Curative is unable to provide you with a replacement CDC vaccine record card.
