How do I cancel or reschedule my appointment?
If you would like to cancel your vaccination appointment, please follow the steps below:
- Go to curative.com/manage
- Enter your Appointment Reference Number and click Look up appointment:
- Click the link that was sent to you by email and/or text
- Enter your date of birth and click Save and Continue:
- At the bottom of the screen, click Cancel appointment:
- If the status shows as Canceled, your appointment was successfully canceled:
Given the limited supply of vaccines, if you choose to cancel your vaccination appointment, we cannot guarantee you will be able to get another appointment.
To reschedule your appointment, please call our support team at 888-702-9042, with the following information handy:
- Full name
- Date of birth
- Email address
- Phone Number
- Vaccination Appointment Number
- Vaccination Site
